Champion Center has contracts with many PPO and HMO insurance plans, and we have highly skilled admissions and billing staff who can answer your questions and smoothly walk you through an otherwise complex process.
When calling, please be prepared to answer the following questions:
- Full Name of the Patient
- Patient’s Date of Birth
- Subscriber’s Name, Relationship to Patient, Employer and Date of Birth
- Name of Insurance Company
- Member ID or Policy number
- Provider’s Phone number
- Type of Insurance Plan
Protecting confidentiality is critical and we follow strict compliance with HIPPA and its privacy regulations.
Once you are admitted to our program, our admissions department and your case managers work together to provide your insurance provider all the information needed to obtain ongoing approval for the multiple phases of our treatment program.
The important part is that we handle all of this for you, so that you and your family can focus on your recovery.
